Ischemic ATN
  • Common type of ATN
  • Most often caused by prerenal axotemia due to hypovolemia
  • any drop is CO can cause Ischemic ATN
  • Pathology
    • ischemia damages the endothelial cells —> decrease GFR
    • ischemia damages the tubule cells —> renal tubular casts/muddy brown casts
    • sites of damage: proximal tubule & thick ascending limb in non-focal pattern
    • rupture of basement membrane
  • Clinical
    • Reversible, but fatal if untreated
    • death most often occurs in early oliguric phase
